    Insurance

I am considered an out-of-network provider. Out-of-network insurance transactions are between the client and the insurer, and the client is responsible to manage this process. I am willing and able to provide clients with a “super bill” receipt detailing dates of service, CPT Codes, DSM-5 diagnoses, and session fees so that clients may submit documentation to insurance companies for potential reimbursement; however, I am not able to guarantee such reimbursement. In all cases, the client is responsible to pay your session fees at the time of service.

I’d recommend asking these questions to your insurance provider to help determine your benefits and potential for reimbursement:

    • Does my health insurance plan include mental health benefits?
    • Do I have a deductible? If so, what is it and have I met it yet?
    • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

    Good Faith Estimate

You have a right to receive a Good Faith Estimate explaining how much your mental health services will cost. Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider for a Good Faith Estimate before you schedule a service, or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, visit www.cms.gov/nosurprises.

    Payment

I accept cash, check and all major credit cards as forms of payment.

    Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
